Abstract: 

Background and aim: Non-melanoma skin cancers (NMSC) are one of the most prevalent malignancies of the human. Although the morality rate is not high for these cancers, they may cause high morbility for the patients. In addition to the skin type, factors such as sun exposure, head and neck radiotherapy, inappropriate use of sunscreen and cultural changes may increase the incidence of these cancers. The objective of this study was to determine NMSC in Isfahan.Materials and Methods: We used data of the cancer resistery center for performing this study and the incidence of NMSC per 100,000 populations was determined. Results: Overall 1681 cases were registered. The incidence of NMSC was calculated to be 10.67/100,000population. The most common site for BCC and SCC was middle face. Conclusion: NMSC incidence in Isfahan is lower than west countries although this incidence is 10.67/100,000in a year. It means that 5-6 thousand cases of NMSC occur in Iran each year. We can prevent its morbidity and mortality by early correct diagnosis.

Abstract: 

Background: Nonmelanoma skin cancer (NMSC) in renal transplant recipients is common and associated with significant morbidity and mortality. The aim of the present systematic review and meta‑ analysis was to estimate the incidence of NMSC among renal transplant recipients. Materials and Methods: We systematically searched PubMed, Medline, Scopus, and Web of Science databases for studies that assessed the incidence of NMSC in renal transplant recipients using a combination of relevant keywords. Two independent investigators included studies and extracted necessary information. Random effect meta‑ analysis was used to estimate pooled incidence of NMSC with 95% confidence intervals (CIs). Results: Twenty‑ nine studies comprising 36, 021 patients meet the criteria for the systematic review. The pooled incidence of NMSC in renal transplant recipients was 12. 6% (95% CI: 12%– 14%) with a majority of squamous cell carcinoma (SCC) 55% (95% CI: 47%– 63%). The pooled estimate of the incidence rates of SCC and basal cell carcinoma was 2. 7% (95% CI: 2%– 3. 4%) and 2. 2% (95% CI: 1. 5%– 2. 8%), respectively. Subgroup analysis per geographic location showed that pooled incidence of NMSC was 39. 1% (95% CI: 26. 3%– 51. 8%), 12. 4% (95% CI: 8. 8%– 16%), and 1. 2% (95% CI: 0. 4%– 2%) in Australia and New Zealand, Europe, and Middle East, respectively. Conclusion: The results of the current meta‑ analysis demonstrated that the incidence of NMSC in renal transplant recipients varies widely. Regarding the high incidence of NMSC among renal transplant recipients, awareness of associated risk factors and early diagnosis of the malignancy in the population is a major clinical need.

Abstract: 

Background: Nonmelanoma skin cancer rates are increasing worldwide. Mohs micrographic surgery and surgical excision (SE) are the two treatment methods for this type of cancer. The current paper aims at determining and comparing the cost-effectiveness of SE and Mohs micrographic surgery. Materials and Methods: The current study has a retrospective cohort design. A number of 630 patients suffering from nonmelanoma skin cancer who at some point of time during the years 2007–2014 referred to the Al-Zahra or Seyed Al-Shohada Hospitals in Isfahan. Patients were followed up for 4 years, and then the incremental cost-effectiveness ratio (ICER) of the two methods was calculated.Results: The average (minimum-maximum) cost of the SE and Mohs surgery methods in Iran was obtained as 18, 550, 170 (2335, 800–260, 898, 262) and 12, 236, 890 (6488, 340–41, 161, 700) Iranian Rial, respectively. Recurrence percentage was also reported as 7.9% and 8.7% for SE and Mohs micrographic surgery, respectively (P>0.05). The ICER of SE in comparison with Mohs surgery was calculated as 7891, 600 Iranian Rials per recurrence avoided.Conclusion: Mohs surgery is less expensive than SE, it seems like Mohs surgery is more affordable, however further studies in different populations of the country are needed.

Abstract: 

Background: The P53 tumor suppressor gene plays important roles in genomic stability. G–to-C at codon 72 in the P53 gene has been associated with increased risk for lung, oral, prostate, breast, colorectal cancers and may be a marker for risk of skin cancer. This project studied the effect of P53 polymorphism on risk of nonmelanoma skin cancers (Squamous and basal cell carcinoma).Methods: This case–control study undertook for study of twenty basal cell carcinoma (BCC), twenty squamous cell carcinoma (SCC) and twenty specimens as controls for each cancer specimen in the city of Isfahan, Iran. Different P53 codon 72 genotypes were identified using polymerase chain reaction.Findings: In control samples, the frequency of Arg/Arg genotype showed 25% and in BCC specimens showed 60%. The differences in this genotype group between the cases and controls were statistically significant (P=0.048). The differences in the frequency of heterozygot Arg/Pro and also in the frequency of Pro/Pro genotype between the cases and controls were not statistically significant. OR=4.5 in BCC cases show that the risk for this disease in persons who have Arg/Arg genotype is about 4.5 times more than normal people. The results of X2 test showed that in this study there is not statistically significant differences between the SCC specimens and controls for frequency of different genotypes.Conclusion: The present study indicate that P53 codon 72 polymorphism is a genetic predisposing factor for risk of nonmelanoma skin cancers (of BCC type) in Isfahan. However, further studies are needed in order to elucidate the role of P53 codon 72 polymorphism in skin cancer development.
